IMPORTANT HEALTH COVERAGE TAX DOCUMENTS 

If you would like a copy of your Form 1095-C or Form 1095-B information statement that UFP has reported to the IRS, please follow the instructions below.  

  1. Log on to mytaxform.com and register for an account using the employer code 12607.
  2. You will enter a user name and password and will be prompted to complete a multi factor authentication. 
  3. During registration, choose “Set me up to receive my tax forms online.” Despite the language on the screen, you can still obtain paper forms with this option. In addition, you can update your form delivery preference anytime in My Account.
  4. On the home page, you will see your available Form 1095 in Recent Tax Forms.
  5. Click the checkbox of the relevant Form 1095.
  6. Then, you can choose to download/view your 1095 or have it sent to you via fax or mail.
